Write a five-line incident summary
Include when and where the event occurred, the type of incident, immediate care, current treatment status, and any insurer or government entity involved.
List the people and organizations
Note the parties, witnesses, medical providers, insurers, employer contacts, and report agencies you know. Mark uncertain spellings or roles.
Gather anchor documents
Have the incident report number, insurance correspondence, important medical instructions, photographs, and a provider list nearby. Do not delay a time-sensitive call to build a perfect packet.
Choose three decision questions
Select the questions most likely to determine fit: relevant experience, the proposed case team, communication, financial terms, or immediate next steps.
Leave space for follow-up
Record who you spoke with, what remains unanswered, what the firm requested, and whether another conversation is needed before either side decides.
